Please arrive for your appointment 10-15 minutes early, so that we can update or verify your current contact information and insurance, and obtain any pertinent clinical information.

If you are new to our practice, please download and complete the patient history forms prior to arriving to your appointment.

Please bring a current list of all your medications.

Inform the clinical staff of any new pertinent medical information to include new diagnoses, allergies, and recent tests from other providers.

If you have diabetes, always bring your blood glucose meter to every diabetes-related appointment, including visits to the RD/CDE. If you wear an insulin pump or sensor, please wear these items to appointments.
